An 8-year-old boy was found to have a right macular abnormality during routine screening. Best-corrected visual acuity was 20/20. Fundus photography showed 2 foveal reflexes (A). OCT angiography revealed an additional area of reduced capillary density temporal to the foveal avascular zone (B). Three-dimensional OCT reconstruction and the corresponding B-scan (C, bottom) showed an accessory depression adjacent to the primary fovea. Microperimetry showed fixation at the primary fovea, with measurable sensitivity near the temporal accessory depression (D).
