Understanding the inflection point marking the onset of diabetic retinal disease (DRD) and the risk factors associated with it remains of preeminent importance. In this issue, Sampani et al1 (p. xxx) report findings from a cohort of 1359 young patients with type 1 diabetes mellitus (T1DM) at a large tertiary referral center between 2005 and 2020. A subset of these patients—69 individuals, covering 133 eyes—also underwent spectral domain-OCT imaging. In their analysis, they found that 5.1% (131 eyes) had DRD, and a small fraction (0.3% or 9 eyes) had diabetic macular edema.
