A 78-year-old woman was referred for a lesion of the left eye. (A) Slit-lamp photography showed a limbal mass exhibiting leukoplakia and a papillary growth pattern, with sessile extension onto the cornea. (B) Concurrent, indirect illumination utilizing the sclerotic scatter technique better demonstrated its organization and the central “hairpin” vessels within each papilla. (C) Excisional biopsy revealed complex papillary proliferation of squamous epithelium; cellular crowding was visible. (D) Higher-power magnification showed atypical cells, with hyperchromatic and enlarged nuclei throughout the entire thickness of the epithelium.
